I would like to express my interest in volunteering as part of the HSkY program to provide support for the Hindu School and/or Classroom activities as needed. I understand that my involvement may include assisting with educational activities, cultural events, and other school-related tasks as requested by the administration or teachers. I agree to comply with all guidelines set forth by the school and maintain a safe, respectful, and supportive environment for all students and staff.
By electronically signing this form below, I the undersigned, state that I am the parent/legal guardian of the student(s) named above. I/We on behalf of, the student(s) registered herein, and, ourselves, do hereby agree to the following: I/We hold, the Hindu School of Kentucky, its staff and volunteers, harmless from any responsibility or liability, for bodily injury claims, property loss or costs arising from any act, or occurrence while attending classes or during participation in any programs or activities conducted under the auspices of the Hindu School of Kentucky. I/We agree to indemnify Hindu School of Kentucky and its representatives, staff and volunteers for costs or liabilities which they may incur as a result of my child/children’s participation in any of its programs. I/We hereby consent to the use of my child/children’s photographs and videos taken during Hindu School events and activities, and waive any rights to privacy in photographs and videos and to compensation related to the use of photographs and videos that may be displayed on websites, promotional material, and social media. I/We fully understand and assume full responsibility for our and our child/children’s actions while entering into this agreement with the Hindu School of Kentucky.
